Primary care plus is a service for people experiencing mental health problems who might benefit from extra support over and above what is available from their GP (family doctor).

We are based at GP practices across the London Borough of Hounslow and work with GPs to provide for people’s mental and physical needs in one place.

The service helps to:

  • Support people who are being discharged from secondary care mental health services, such as a community mental health team.
  • Prevent the need for referral to secondary care.


  • Help that is close to home.
  • Recovery focused support.
  • Partnership with your GP, which improves overall physical and mental wellbeing.

We can help you identify your needs, manage your mental health and help you find practical solutions to life’s challenges. If appropriate, we may tell you about other services that you might find useful.

If you become unwell or need extra support we can arrange help from specialist mental health services.

Meet the team

Jenny Ball

After qualifying from the University of Surrey in 2008, Jenny began her career in specialist forensic services, developing in-depth knowledge and in complex risk management. Jenny went on to develop her skills in acute in-patient settings and subsequently undertook a role with the early intervention service. She is currently completing a BSc in psychosocial interventions for psychosis.

Jenny was a founding member of the Hounslow primary care plus team. She has taken special interest in ensuring all service developments are co-produced, along the way setting up the primary care service user forum, now known as The Wellbeing Exchange. Mukaddes Demirci

Mukaddes qualified in Mental Health Nursing and graduated from St George’s University of London in 2010. She began working as a staff nurse for inpatient services on a busy, acute admissions ward, before taking on the role of community psychiatric nurse, where she worked with the home treatment and crisis resolution team. Mukaddes then worked as a care co-ordinator in community mental health service for older people, supporting people with age related mental health problems and complex needs.

Emma Brown

Emma qualified from Buckinghamshire New University (BNU) with a BSc in Mental Health Nursing in 2011. She is currently completing an MSc in psychotherapeutic approaches to mental health also at BNU. Emma has worked in various areas of mental health with a specific focus on inpatient services.

Emma was the clinical team leader on a busy admission ward in Ealing before coming to primary care plus. Emma is a registered mentor and the lead on student nurse education for the service. Emma is also responsible for GP training in Hounslow. Emma covers Chiswick and can be contacted at or on 07813 537 811.

Helen Potter

Helen graduated from Buckingham Chiltern University in 2000. After spending three years on inpatient settings she undertook numerous roles in diverse community settings including community mental health teams (now recovery services) and more intense assessment work where she was acting lead practitioner for the Ealing assessment service.

Helen has been lead practitioner of the Hounslow primary care plus team since it was set up and has been instrumental in its development. As well as covering her duties as team leader, she covers the GP surgeries within the Great West Road are and can be contacted at or on 07813 537 808.

Kieran Leen

Kieran qualified from Tralee Institute of Technology with a BSc in psychiatric nursing in 2007. He has since completed an MSc in social research methods at Teesside University and has worked in a variety of diverse areas of mental health care. These include working at a high support day hospital in North Dublin, inpatient assessment units in south Dublin and west London. He also worked for 21/2 years as a care co-ordinator at recovery team south in Hammersmith & Fulham.

Kieran helped develop the Hammersmith & Fulham primary care mental health service before moving to Hounslow to join primary care plus.
